While operating in an underground coal mine at Wongawilli Colliery near Wollongong on 4 July 2017, a catastrophic failure of a diesel engine system occurred. This resulted in internal engine components being ejected from a cylinder block and being exposed to the mine atmosphere. The temperature of the ejected components created an ignition source which had the potential to cause an explosion. However, no one was injured as a result of the incident.
