Near Miss: Understanding the Risk and Best Practices to Prevent
Attached is an impressive flyer describing a near miss truck collision. The flyer, developed by District 5, may be useful at your mine. You may want to add a bullet about establishing traffic control procedures – particularly at critical points such as switchbacks.
Click Here for the flyer which details an accident which occurred at approximately 1:15 a.m. when a 789 hauler was traveling up an approximate 4% grade. While negotiating a right switchback, the 789 hauler was struck in the left fron side by a 785 hauler traveling downgrade in the switchback. This collision resulted in an immediate fire in the engine compartment area of the 789 hauler resulting in loss of both haulers.
