We’ve all heard “question everything” before. Some might think that’s unnecessary or disrespectful; while others think it’s necessary and helpful. Where do we fall regarding this as it relates to safety? I think we’d all agree that in order to be perfect at something we need to understand not only what is supposed to happen, but also what could happen during operation and/or failure. If we don’t ask the questions, we won’t know. When we don’t know – we error with things like “eyes and minds not on task, line of fire, and balance, traction, or grip. Those errors enable injuries to occur.
So it all starts with a question. Do we want safety perfection? If so, let’s begin today…
