This is a form we used for many years. It requires electrical warning stickers for equipment which may work around or in proximity to electrical equipment, such as cranes. In addition it requires that the user develop and use a sticker which is similar to a state vehicle inspection sticker. This allows drive by confirmation as to whether or not a piece of equipment was inspected prior to being used on company property.

One item missing is pinch points labels for articulating equipment, but the manufacturer usually provides those during assembly. It could be added as a line item.
