The Hazard of Falling Objects:

Here are some materials that can be used as a safety meeting. They address the hazard of falling materials. Each of the events described could easily have been a fatality. Learning from events like this and changing our practices to address these learnings will help us reach our goal of no fatalities.  In recent times within BHPB there have been a number of Significant Incidents involving Dropped Objects.

Attached here is a one page summary of 4 of these incidents, their causes and the key learning’s for BHPB. Links to the full incident reports are  included in the notice. Included with the summary are Coach and Audit sheets for controlling dropped objects. The Coach sheet can be used as a training guide and the Audit sheet provides a checklist when conducting Audits, Reviews and
Critical Task Observations around dropped objects. Please review these incidents for similar potential at your site and share with your contractors. You may also find this information valuable for tool box talks, risk reviews and general safety communications.
