Do our hazard controls ever create additional hazards? Does what we do to fix something ever needs fixing in itself? Safety is ensuring we not only fix the hazard, but we also prevent any new hazards from arising. These pictures were shared by A S during his recent trip to the Ukraine where he saw these plywood manhole covers and thought we’d appreciate the ineffective hazard controls. The effort was made to cover the hazard when in reality it created yet additional ones. As safety champions, we all need to make certain we’re considering new hazards while controlling the existing ones.
