These pictures were sent to share to show the importance of pre-job safety, preventive maintenance, and inspection before using equipment.
We may want to know what happened next, but that is a “cliff hanger”.
What we want to ask ourselves is: “What happened first to get to the root cause to prevent this from happening again.
FOCUS: Inspect equipment before use.
Every Day Is An Opportunity To Learn Something New!
Sharing several pictures of a W/O rig with a brake failure while it was navigating a windy mountain road. Pictures clearly tell a story…….PM – is one of your barriers that cannot be short-cutted! No specific details to share, but I’m wondering what brake was completed on location?
A picture is worth a thousand words.
This rig was working at (location deleted). Brake work had been completed on location just before this incident.
Good example for vehicle safety discussion.
This would be a good safety topic to include in our monthly rig safety meetings. The rig was working for (company name deleted) coming off of (location deleted)-Happened Thursday of last week. This illustrates how crucial equipment inspection is prior to coming up and going down the mountain.
Thanks for the share KJ!
