MSHA Fatal #12 – Machinery – SUR – Vermont
Preliminary: On Wednesday, July 17, 2019, a 32-year-old general manager with 14 years mining experience and a coworker was preparing to replace a drive motor on the primary crusher when a hydraulic hammer fell off an excavator, striking the victim in the head, and pinning him against the crushing plant.
FYI: Fatal #7 that occurred on May 13 was deemed non-chargeable on June 26, 2019; therefore, the fatal count is 11 total mining fatalities to date (not 12). However, I will continue to use the fatal reference number that will be found on preliminary and final reports which will reflect the total at the time the fatal occurred.
